Venous thromboembolism (VTE), including deep vein thrombosis and pulmonary embolism, commonly occurs in patients undergoing rehabilitative therapy for various conditions and is an important healthcare problem resulting in significant morbidity and mortality. Despite evidence-based guidelines for the prevention of VTE, thromboprophylaxis is underused in clinical practice. A recent study observed low rates of compliance with guidelines for thromboprophylaxis associated with the Sixth American College of Chest Physicians Conference on Antithrombotic Therapy. The need to promote the use of evidence-based guidelines for thromboprophylaxis extends to at-risk patients in rehabilitation clinics, as 1 retrospective study found that only 38% of patients admitted to a rehabilitation hospital received antithrombotic therapy. Barriers responsible for poor compliance with current thromboprophylaxis guidelines include lack of familiarity with recommendations, overestimation of bleeding risk, underestimation of thrombotic risk, and logistical limitations. This failure can be corrected by development of a heightened awareness of risk factors and understanding of the current evidence-based treatment and prevention guidelines associated with VTE management among all healthcare professionals on staff at rehabilitation clinics.
